If you are a new consumer enrolling in one of the TennCare Consumer Directed Programs, your Supports Broker will be reaching out to you about the enrollment process, including the use of the online enrollment tool. If you are currently enrolled in one of the TennCare Consumer Directed Programs and receiving services, contact your Supports Broker, and they can help walk you through the online enrollment process.
Every year, TennCare, DIDD and PPL work together to develop and release the Annual Satisfaction Survey for Participants in the Self-Determination Waiver Program (SDWP).
This year, the surveys are also available to complete anonymously online! Surveys are mailed out to Participants for anonymous response.
Please only complete the survey once. The survey will close 11/01/2021.
